Making sense of your medicine list
A simple way to keep track of everything you take, and what to bring to appointments.

Many people take more than one medicine, plus vitamins and the occasional remedy. A single written list takes the guesswork out of every appointment.
Write down each item, what it is for, how much you take and when. Add anything you take only now and then. Keep a copy in your wallet and one on the fridge.
Bring the list to your pharmacist and your prescriber. Ask whether anything can be simplified or timed to fit your day better. If you would like help, our medication review is free for first-time visitors.
This guide is general information, not personal medical advice. Ask your pharmacist about your own situation.
